To Consider Alternatives on Streamlining the Review Process for Increases of Floor Area Ratio. This Proposal is Part of the Planning Division's Work Plan Approved by the City Council for the 2004-2005 Fiscal Year

AWARD CONSTRUCTION CONTRACT FOR FREMONT BOULEVARD MEDIAN LANDSCAPING, CITY PROJECT 8384 LANDSCAPING, CITY PROJECT 8384(PWC)

Approval of Plans and Specifications and Award of Contract to the Lowest Responsible Bidder for the Landscape Installation at Fremont Boulevard Medians Between Mowry Avenue and Sundale Drive; City Project No.8384(PWC), Fremont Boulevard Median Landscaping

RECOMMENDATION:

1.  Approve the plans and specifications for Fremont Boulevard Median Landscaping, City Project No. 8384(PWC).

2.  Add the installation of 6,000 square feet of cobble to the Stevenson Boulevard (I-880) Overcrossing median (known as Add Alternate A) to Fremont Boulevard Median Landscaping, City Project No. 8384.

3.  Accept the bid and award the contract to Indio's Construction, Inc., as the lowest responsible bidder in the amount of $350,000 and authorize the City Manager to sign the contract.

GRANT APPLICATION FOR LOCAL STREETS AND ROADS REHABILITATION

Resolution Authorizing Staff to File Grant Application for Federal Surface Transportation Program (STP) and Congestion Mitigation and Air Quality Improvement Program (CMAQ)

RECOMMENDATION:

1.  Adopt a resolution authorizing the filing of an application for Federal Surface Transportation Program Funding for street overlay project and committing the necessary non-federal match for the project and stating the assurance of the City of Fremont to complete the project.

2.  Upon final approval of funding, authorize the City Manager to accept the grant, and appropriate those funds to the street overlay project.

APPROPRIATION OF ANTICIPATED REVENUES FOR COMPLETION OF THE NEW MAINTENANCE CENTER

  Approve Appropriation of Anticipated Revenues to Facilitate Completion of New Maintenance Center Project (PWC 8038) in the Amount of $586,000 and Appropriate Interest Revenue from Debt Fund to Project Fund PWC 8038 in the Amount of $230,000

Authorization for the City Manager to Negotiate and Execute a Memorandum of Understanding with Alameda County Health Care Services Agency for Fremont Family Resource Center Medi-Cal Targeted Case Management

RECOMMENDATION: Authorize the City Manager to negotiate and execute a Memorandum of Understanding with the County of Alameda for 2004/05, to enable the Human Services Department to receive up to $158,884 to provide case management to local Medi-Cal eligible individuals and their families as part of the Targeted Case Management program.

Authorization for the City Manager to Negotiate and Execute a Memorandum of Understanding with Alameda County Health Care Services Agency for Fremont Aging and Family Services' Medi-Cal Targeted Case Management

RECOMMENDATION: Authorize the City Manager to negotiate and execute a Memorandum of Understanding with Alameda County for 2004/05, to enable the Human Services Department to receive up to $76,407 to provide case management to Medi-Cal eligible adult clients as part of the Targeted Case Management program.
