Description of Agencies and Programs

Afghan Coalition

A community-based organization that helps the refugee community in the Fremont area to have access to all services that are available to them. This agency offers programs such as support group for women, youth leadership program, community training, community events, health navigation and advocacy, mental health services, case management, I&R, and youth tutoring and mentorship.

Alameda County Tri-City Community Support Center

A division of the County's Behavioral Services Agency and offers outpatient mental health services for adults with chronic, persistent mental health needs. Services include case management, screening and referrals to hospitalization and socialization programs. To obtain services, use ACCESS Line 1-800-491-9099.

Alameda County Public Health Department-Public Health Nursing

Public Health Nursing provides nurses to assess the health status and provide health education, case management, counseling, outreach and referral services for the individuals, families and community groups. Specific populations include high-risk pregnant and parenting women, infants and children, people with or exposed to communicable diseases and the elderly.

Alameda County Social Services - Department of Economic Benefits

The Department of Economic Benefits is within the County's Social Services Agency and provides financial assistance and services to needy families with children under 18 years of age who meet financial requirements for CalWORKs to receive employment and community self sufficiency services to assist families in getting off welfare and into the labor market.
