What is Senior Support Services?

We are a team of caring professionals in the City of Fremont Human Services Department. Our job is to help seniors get services to improve their safety, well-being, and health, so that they can stay independent in their homes.

We have diverse backgrounds in social work, nursing, psychology and gerontology. We know how to help seniors. We have worked for years with other senior-serving agencies, and can help you connect with various services you want.

Services We Can Help You Obtain Include

  • Someone to help you in an emergency.
  • Someone to help you with household chores or errands.
  • Hot meals delivered to your home.
  • Someone to talk with on a regular basis.
  • Medical care.
  • Transportation to doctors' appointments.
  • Help with money or making ends meet.
  • Recreational or social activities.
  • Help with legal problems.
  • And others...

Who is Eligible for Our Assistance?

 
We will help any senior over 60 years old who lives in the cities of Fremont, Newark or Union City.

What is the Cost of Our Assistance?

 
Donations are appreciated to support this program, but no one will be denied our help due to inability to donate. Senior Support Services is funded by the cities of Fremont, Newark and Union City; Older Americans Act funds provided by the Alameda County Area Agency on Aging; and through community contributions.

With just one phone call, you will get all the help you need. Our staff-member will talk with you to:

Assess

 
Discuss with you your needs and concerns.

Plan and Coordinate Services


Inform you of available care and services, and help you get the services you need.

Advocate

Speak to other agencies on your behalf to obtain services and benefits to which you may be entitled, such as income assistance, health coverage, or housing.

Monitor

Follow-up to make sure that services are serving you effectively. We will help you find solutions to problems.

Multi-Purpose Senior Services Program (MSSP)

The Multi-Purpose Senior Services Program is a leading provider of case management services. Our goal is to help frail seniors remain at home by arranging for needed services. 
 
As we age, it can become more challenging to manage at home.  Health conditions and other factors can make routine tasks like cooking, laundry, going to the doctor, and paying bills seem overwhelming. 
The Multi-Purpose Senior Services Program (MSSP) is designed to enable seniors to remain safe and independent in their own homes.

What to Expect

  • A home visit by a social worker and a public health nurse.
  • A care plan developed with the senior, his/her physician, family, and other caregivers to address the senior's total needs (physical, emotional, social, and environmental).
  • Referral to and purchase of services that enable the senior to remain safe and independent at home.

Paid for by Medi-Cal - MSSP services for seniors enrolled in the program are covered by Medi-Cal

Please call us now if you are:
  • 65 years old or older
  • Receiving Medi-Cal benefits with no or minimal share of cost
  • At risk of institutionalization (certifiable for care in a skilled-nursing or intermediate-care facility)
  • Residing in Fremont, Newark, Union City, or Hayward
