• Approximate time of Incident
• Address/location of Incident

If you are seeking a copy of a Pre-Hospital Patient Care Report, please scroll to the appropriate section below for the process. This section is only for requesting an Incident Report.

• CUPA (Certified Unified Program Agencies)
• Underground storage tanks
• Fire Code Violations, Fire Inspections, etc.

Pre-Hospital Patient Care records are protected by Health Insurance Portability and Accountability Act (HIPAA) and cannot be requested through the City's Public Records website.

Request Process

If the requestor is the person/patient involved in the incident:

• Complete the Authorization for Release of Patient Care Records form
• Email it to Fremont Fire Department, fremontfire@fremont.gov 
• If the requestor is the person/patient involved in the incident no supporting documents are needed
• You will be notified by email if the record(s) have been identified and processed for release or if we are unable to locate the record
• The records (Incident Report and the Pre-Hospital Patient Care Report) will be released as noted in the Authorization form

If the requestor is a third party (not the person/patient involved in the incident): 

• Complete the Authorization for Release of Patient Care Records form
• Email it to Fremont Fire Department, fremontfire@fremont.gov
• Provide HIPAA compliant supporting documentation (this allows the Fire Department to legally release the Patient Care Report to a third party)
• You will be notified by email if the record(s) have been identified and processed for release or if we are unable to locate the record
• The records (Incident Report and the Pre-Hospital Patient Care Report) will be released as noted in the Authorization form.

     Or Submit a Subpoena Request

• Issue a Subpoena Request to the Fire Department via the City Clerk's office
• Subpoenas can be hand-delivered or sent via postal mail. We cannot accept email requests.
• If you are hand-delivering your subpoena, email the City Clerk's Office to schedule a delivery appointment.

To request a member of the Fremont Fire Department to appear at a deposition, trial, legal hearing, testimony, etc., please follow process below:

• Issue a Subpoena Request to the Fremont Fire Department with the name of the employee whose presence is being requested.

• To avoid delays and ensure efficiency, include location, date, and time of the incident associated with your request plus the applicable Subpoena check (civil cases) made payable to the City of Fremont.

• Subpoenas for personnel appearance can be hand delivered or sent via postal mail to Custodian of Records, Fremont Fire Department. We cannot accept email requests.
