The Family Support Services program offers support to families facing stressful or challenging life situations. A professional social worker can assist you in getting through difficult life situations. Services are available to Tri-City residents and free to those with qualifying incomes.

A Family Support Coordinator will work with you and your family to develop an action plan based on your goals. You will receive information about options, tools, and access to resources needed to resolve or stabilize your situation.

Services Available:

• Assessments and Comprehensive Service Planning
• Home or office visits to discuss family needs, goals, and resources
• Consultation as how to best meet those needs
• Service linkage and coordination with other care providers
• Follow-up with you and your family
• Information and referrals
• Consultation and advocacy with other care providers on your behalf
• Ongoing case management support
