The following documents are required to prepare your tax returns. Bring the following documents to your tax appointment:
- A Social Security card and/or ITIN for EACH person listed on the tax return.
- In lieu of a Social Security card, the following documents are also accepted:
- An SSA-1099 form.
- A prior year’s tax return, if and only if it was prepared by the Fremont VITA program and every person that will be listed on the current year’s return was also listed on that prior tax return.
- Valid (unexpired) photo ID for the main tax filer and spouse.
- If Married Filing Jointly, both spouses must be present at the tax appointment.
- Please bring the following tax documents as applicable:
- W-2, Wage and Tax Statement
- W-2G, Certain Gambling Winnings
- SSA-1099, Social Security Benefit Statement
- 1095-A, Health Insurance Marketplace Statement
- 1098, Mortgage Interest Statement
- 1098-E, Student Loan Interest Statement
- 1098-T, Tuition Statement
- If you are bringing a Form 1098-T, please also provide a detailed student account statement of expenses paid to the university/college.
- 1099-B, Proceeds from Broker and Barter Exchange Transactions
- 1099-C, Cancellation of Debt
- 1099-DIV, Dividends and Distributions
- 1099-G, Certain Government Payments
- 1099-INT, Interest Income
- 1099-K, Payment Cared and Third Party Network Transactions
- 1099-MISC, Miscellaneous Information
- 1099-NEC, Nonemployee Compensation
- 1099-R, Distributions from Pensions, Annuities, Retirement or Profit-Sharing Plans, IRAs, Insurance Contracts, etc.
- Identity Theft PIN Letter from IRS
- Records of charitable contributions, such as receipts or letters from the charity
- Property tax bills
- DMV registration form
- Childcare or Dependent Care Expenses - total amount paid, provider's business name, address, phone number, and SSN or tax ID number.
If you are self-employed, provide business income and expenses, including mileage log. Rideshare Drivers should provide their 1099-NEC, 1099-K and online Tax Summary page.
