Aide/Behavioral Specialist Form: Does your child have an aide or specialist that will attend with them at their registered camp/class? Submit this form. Aides/Specialists must be background checked and TB tested.  Please submit this form at least 7 calendar days before the start of the activity.

Refund/Transfer Request Form: Submit your request for a refund or transfer through this form.

Need to request a refund or transfer for an activity? Complete this form to request a refund or transfer. See the policy below.

• REFUNDS are ONLY allowed if requested up to seven (7) calendar days prior to the start of the activity.  Unfortunately, there are no exceptions to this rule, including; illness, injury, family emergencies, scheduling conflicts, forgetting about or missing classes, or any other reason not listed.  Request for refunds must be received in writing via online form at this link.  A $10 withdrawal fee, per class/activity, will be deducted from your refund if received (7) calendar days prior to the start of the activity and approved; and all refunds will be refunded to the payment method/credit card used for the original purchase.  Please allow up to four weeks for receipt if refunded by check (if paid with cash or check).

• TRANSFERS are allowed up to seven (7) calendar days prior to the start of your currently enrolled activity. There is no charge for this service. Request for transfer must be received in writing via online form at this link.

• CANCELLATIONS: We reserve the right to cancel an activity for any reasonable cause. Should this happen, you will be refunded in full, or you may transfer to any open activity.
